The Collaborative health partnership

The Collaborative is a partnership between the Royal Melbourne Hospital, North Western Melbourne Primary Health Network, cohealth, and Merri Health. Formed in 2012, it covers the geographical areas of Melbourne, Moreland, Moonee Valley and Yarra local government authorities (LGAs).

Our communities are living for longer and more of us are getting chronic illnesses. The Collaborative addresses issues like this by working together to see how we can provide people with the right care, at the right time, and in the right place.

We want to support people to improve their health and wellbeing, so they don’t need to visit hospital.

 Our projects have been focused on:

  • Back pain
  • Chronic Health Failure
  • Chronic Kidney Disease
  • Diabetes
  • Digital Health
  • Advance care planning

The Collaborative focused on improving care pathways for people who experience Dyspnoea (shortness of breath/difficulty breathing).

This service is now managed by existing cohealth and Merri Health services. It is available for people with any chronic illness, including COPD and heart failure. 

You will receive the same model of care, with a new referral route.
