Education and exercise for people living with chronic conditions.
What is it?
Be Well, Live Well is an 8 week program suitable for people who have, or are at risk of, chronic cardiac or respiratory disease or diabetes, or people who need to change their lifestyle.
The program provides group and individual education and exercise that supports you to:
- Stay as healthy and well as possible
- Build confidence and knowledge to make healthier choices
- Feel better again after being unwell or in hospital
Carers and support people are welcome.
Sessions are currently being conducted online via telehealth with some onsite exceptions.
Who can use this service?
Anyone with a chronic condition.
Cost
There is a small fee for some of our services. Some of our services are free.

Do I need a referral?
No, you do not need a referral to access this service, but you need to have a pre assessment with a care coordinator and exercise physiologist first.
