Sign up to Carer Gateway through our self-referral form
Victorian carers can now register for Carer Gateway services using our new self-referral form! Simply fill out the form to register your caring role and request a callback. A member of the Carer Gateway team will then contact you to complete the registration process.
E-referrals for Carer Gateway:
Carer Gateway has developed a new method for health professionals to refer carers to the free service, with referrals now able to be made online simply and safety.
By referring a Carer onto the Carer Gateway, you will help the Carer to:
- access practical information and advice
- access services and supports they need to succeed in their caring role
- access free counselling and coaching services
- connect with others in a similar caring role
