Carer Gateway has developed a new method for health professionals to refer carers to this free Australian Government service, with referrals now able to be made online simply and safely.
Carer Gateway supports anyone who is an unpaid carer looking after someone with a disability, mental illness, a medical condition (including terminal or chronic illness) or who is frail because they are aged.
Carers can be any age, providing any sort of care. Some carers may look after another person 24 hours a day and help with daily living, while others may look after people who are relatively independent but sometimes need help.
Carer Gateway are committed to fostering healthy and connected communities and strive to continuously improve operational processes to deliver high quality services to carers.
The new electronic referral (eReferral) system will provide a simple process for referrals from health and wellbeing service providers, along with increasing carer referrals by ensuring the new method is easy.
The system also aims to improve operational efficiency in receiving and processing referrals.
Carer Gateway service provides carers with access to:
- practical information and advice
- services and supports they need to enhance their caring role
- free counselling
- respite
- coaching services
- targeted financial assistance packages
- other carers in similar caring roles to connect to
Carer Gateway is delivered in Victoria by a consortium of partners led by Merri Health. This group includes Alfred Health, Ballarat Health Services, Barwon Health, Bendigo Health Care Group, FamilyCare, and Uniting (Victoria and Tasmania) Ltd.
