We offer healthy eating and nutritional advice for people with a diet-related health concern, such as diabetes, being under- or overweight, chronic disease, gastrointestinal problems, cancer and allergies.

Exercise physiology helps support people to stay active and maintain their health and mobility. Exercise is a proven tool to help prevent and treat chronic health conditions such as heart disease, diabetes, dementia and osteoarthritis.

Physical inactivity is a risk factor for developing chronic conditions and 90% of adults aren’t meeting Australian Physical Activity Guidelines. Our exercise physiologists are here to help change this.

Following a comprehensive assessment of a person’s health status, current activity levels and goals, our exercise physiologists will develop an exercise plan to teach clients how to exercise safely at home – as well as at local leisure centres, pools and gyms. Exercise programs are tailored to individual needs and goals.

We also deliver group exercise programs and hydrotherapy sessions at our Fawkner site.

Well@Home’s kind, friendly and approachable nurses provide general clinical assessments to identify healthcare needs, such as medication management, vital sign monitoring, and a range of other health needs. They can also provide dementia assessments and continence assessments (to review elimination and continence needs/concerns).

Our nursing staff are culturally sensitive and flexible in how they provide care, in line with their clients’ personal preferences.

They also provide education, funding application support and referral to other services where needed.

Occupational therapists (OTs) provide support to complete activities safely and independently. These activities may include self-care (e.g. showering and dressing), domestic tasks (cooking and cleaning), or leisure activities (socialising and accessing the local community).

Our OTs can help people adapt the way they do things and modify their home environment and can also prescribe equipment to support them to reach their goals.

Our physiotherapists can help manage gait and mobility issues, falls and balance issues, and can assist with pain management and recovery following musculoskeletal injuries or surgery. They provide tailored exercise programs, gait aid prescriptions, and education regarding falls prevention and pain management.

We offer support to prevent and treat foot, ankle and lower leg problems. Our podiatrists can provide information on how to avoid problems as well as give advice on foot care and footwear.

We assist with communication and swallowing problems, as well as help to diagnose and treat these problems, including:

  • speech and language difficulties
  • general communication
  • swallowing problems like choking or coughing on food
  • memory problems
  • voice problems.
