YOUR NEW ROLE

• This role is your chance to join the highly skilled HARP Complex Care service at Merri Health to provide coordinated, goal orientated care to clients with heart failure.
• You will work closely with the cardiology department at RMH to help improve client’s quality of life and reduce avoidable hospital admissions.
• You will be encouraged to share ideas through quality improvement initiatives and supported with ongoing professional development.

YOU WILL BE RESPONSIBLE FOR

As a HARP Heart Failure Nurse, you will be responsible for:
• Clinical care for clients with Heart Failure including chronic heart failure specialist assessment, education, care planning and referrals.
• Work closely with clients, their carers, hospital clinicians and GPs to provide integrated client-centred care for clients with heart failure in accordance with best practice guidelines.
• Improve self-management and health outcomes of clients through provision of education on anatomy and physiology of heart disease conditions, symptom recognition and management, medication management, exercise and exercise tolerance, advanced clinical assessment and exacerbation management.
• Collaborate with HARP multi-disciplinary team members to identify and address client’s psycho-social, behavioural and environmental needs associated with their complex conditions

To be successful with Merri you will go above and beyond to achieve great results for our clients, the organisation and yourself. We’re looking for individuals with:
• Registered Nurse Division 1 and a current registration with the Australian Health Practitioner Regulation Agency (APHRA).
• Post graduate qualification in Cardiac Care, Critical or Acute care or equivalent experience
• Demonstrated experience in client centered care through effective cardiac assessment, care planning, case conferencing review and escalation of care for clients.
• Demonstrated ability to work autonomously and collaboratively as part of a multi-disciplinary team.
• Strong organisational, planning, time management and prioritisation skills
