Your new role
Occupational Therapists funded under the Home and Community Care Program for Younger People (HACCPYP) program provide services to clients under the age of 65 (under 50 for Aboriginal and Torres Strait Islander people), some of whom have a functional disability that risks a loss of independence. Being HACCPYP funded, the role aims to maximise clients’ and carers’ functional independence and their ability to participate fully in the community.
The role also has a particular focus on marginalized and disadvantaged communities.
This position interacts closely with the multi-disciplinary team and other service providers, both internally and externally. It also provides opportunities to provide group education sessions and be involved in developing innovative service delivery options to service our community. Roles may also include working to provide NDIS functional capacity assessments alongside the wider multi-disciplinary team.
You will be responsible for
High quality individualized assessment and management of clients using a strengths-based approach - including home modifications, equipment prescription, fatigue and sleep management and other educational interventions
Assessments and interventions performed through home and community visiting and site-based avenues
Completion of relevant documentation (eg SWEP applications, housing advocacy, NDIS functional capacity assessments alongside our NDIS assessment team)
Participation in delivery of group education sessions
Participation in multidisciplinary care co-ordination, treatment planning and goal direct care planning for client care
Attendance and participation in regular clinical support meetings & quality project planning
Opportunity to be involved in OT student placement planning, supervision and feedback.
To succeed in this role you will have
Current Occupational Therapist registration with AHPRA.
Tertiary qualification in Occupational Therapy
Minimum 2 years’ experience working as an Occupational Therapist
Encourage experienced Grade 1's to apply to work in a dynamic and well-supported community health environment passionate about providing values based healthcare to our clients
