Improve your long-term health supported by an expert team

If you have a chronic health condition like diabetes, heart disease, cancer, or arthritis, or if you have risk factors such as high blood pressure, cholesterol, or excess weight, we are here to support you in making long-term lifestyle changes to improve your health and wellbeing.

Our program runs for 8 sessions, each lasting 2 hours. You'll be guided and supported by a team of experienced professionals, including a Physiotherapist, Dietitian, Nurse, Occupational Therapist, and an Exercise Physiologist. Together, we'll provide you with valuable information, engaging discussions, and the opportunity to learn new skills, including an introduction to different types of exercises.

Where and When:

Merri Health, 93 Bell Street, Coburg

Mondays from 1:00 pm to 3:00 pm: 

  • September 4th, 11th
  • October 2nd, 16th, 23rd, 30th
  • November 13th, 27th, 2023 

To ensure the program is tailored to your needs, we'll conduct an assessment before it starts, either in late July or August.

What to Bring:

Please wear comfortable clothing and shoes suitable for physical activities. Don't forget to bring a water bottle to stay hydrated during the sessions. If you require any medications or devices before, during, or after exercise, please have them with you.

Cost:

The program fee is only $5 for concession cardholders or $8 for others.

What can participants expect:

At Be Well Live Well, our goal is to empower you to make lasting changes that positively impact your health and wellbeing. We don't just talk about problems and give advice; instead, we take an active approach using evidence-based behaviour change techniques, including motivational interviewing (MI), goal setting, action planning, and elements of Acceptance and Commitment Therapy (ACT).

Throughout the program, we'll help you learn new skills to overcome barriers and better care for your health while also acknowledging that some things may be beyond your personal control. We'll engage in discussions about your values and what matters most to you, identifying your strengths and qualities to take steps toward a healthier life.

After each session, we encourage you to develop an action plan, a specific and practical step you can take to actively improve your health. We understand that change can be challenging and uncomfortable at times, but rest assured that our team is here to support you every step of the way.

As part of the program, we'll explore mindfulness and various physical activities that may be new or challenging for you. Please remember that participation in these activities is entirely optional, and you are free to say no or sit out at any time.
